PDF TOC and Index page titles are missing

This is about PDF output. I have one project that shows the title 'Contents' on top of the TOC page and the title 'Index' on top of the index page. Other project do not show these titles. I have searched for the differences to find where these titles can be added, but I don't find the difference. Can anyone help me please?

    Same CSS being used for both projects?

    Yes, the CSS are the same.

    Strangly, when I recreated the PDF of the 'good' project, the titles were gone. ???

     

    But, I found a solution: In the toc.plt and index.plt, I added <h1>Contents</h1> to the div of the content.
