after filling in pdf form, error message page is what appears on file saved

I follow the instruction on upper right part of frame and this:

So I choose to open with Adobe Reader and all is well until I save the form. When people try to open the forms, all they see is the page with the "PLEASE WAIT" message and no saved PDF form. I have also used the option of saving file but same thing happens...no saved PDF forms are seen. 😞  Help please??  Thank you.
