New bug? No icon to insert images

When I now click the blue Reply button, I have no icon to insert images in the post.  Only the insert video icon is showing. 

I am only able to post this screenshot by click and drag (then double-clicking the image to open up the sizing options).  A user was complaining that he could not find the insert photo icon that I had given him a screenshot for.  I thought he was not clicking the blue Reply button, but he insisted he was.  The only way he was able to post a screenshot was to create a new post.  Adobe, please add this to my bill for wasted time.
