@Fred38914109xiqu essentially, you can go back to the original file that was saved by the person who supplied it (aka Fivrr) and open that PDF, 'edit PDF' and select the telephone icon, copy it and then bring it back to the other PDF that you are working on and paste it. 

Also, chances are it was a font substitution, the person may be using a MAC font such as Wingdings or Webdings or Symbols.

How did you originally created this file in what application?
And in the original document, it appeared as properly?
And did you export or save the file as a PDF? 

Mac or PC?

It was originally created in Adobe by someone on Fiverr. Once I got an
Adobe account, I uploaded it.
It was saved as a PDF.
It didn't look like that at first, but I've made changes and saved it.
I use Windows on a PC.
Note: Through playing around with it, I believe that I moved the telephone
icon and didn't realize it. I deleted it, but now that I see how it works,
I want to put the icon back into my document. How would I find the
telephone icon?
