Clear Form button

How do I add a "Clear button" to a form that will clear all entries in drop downs, text fields, calendar drop downs, etc.

Adobe Acrobat Reader cannot edit forms.

If you have Acrobat you can add a button and set its Mouse Up action to "Reset form". This is not possible in Reader, as mentioned.
