Form Data gets duplicated

I have document  with 5 pages (1,2,3,,4,5) and each page contains fields that need to be filed. On page 2 I needed more space so I extracted page 2 using some online tools and inserted it into my document so now I have 1,2,2,3,4,5 which is exactly what i wanted. Now problem I have is that when I am typing data into page 2 data gets copied into another page 2. How would I resolve this issue?

You must change the name of the form fields. This is not possible with Acrobat Reader.

What about adobe pro dc?
