Table Of Content is created in InDesign.
Does pdf have TOC in it?
Also, let me know application that you are using to view the PDF, is it Acrobat or Reader?

Yes, there are pages of TOC in the pdf.
In fact, using Preview in iOS, I am able to see the TOC.

Unfortunately, it is book licensed for use by myself only. Sending only the TOC pages to you would not be useful would it?

Do you mean the bookmarks? If so, that tab will only appear if the file actually has them (which it doesn't necessarily have).

In the document I'm reading, a 1071 page math text, the table of contents in Adobe Reader DC (v2015.016 ) is automatically constructed and made available as a hierarchical set of Bookmarks, which appear to be functionally and visually identical to that provided by Preview (v859.21) on Mac OSX (v10.10.5).
Note: The table of contents also appears as pages of the text itself, but it does not have clickable links, so an in-text TOC might be a requirement for a bookmark based TOC.

No, the two things have nothing to do with each other. You can have a bookmarks tree with or without a TOC (clickable or not), and vice versa.

Clearly, I misinterpreted what I experienced on displaying this particular document in Adobe Reader. It must have already contained a bookmark implementation of a TOC. I will try to delete my comment.

I can see the table of content in some pdf viewers; for example Microsoft app:
but not in Adobe acrobat pro, there I see the content of each page):
How can I set adobe acrobat pro to see the table of contents?
