How do I remove ALL images from a .pdf document?

You can't with Reader. You may be able to with Acrobat, depending on the document's security restrictions.

Provided the security restrictions of the document permit, copy the text and paste it in a word processing application. Recompose and produce a new PDF if desired. This way you will not be left with the blanks you'll get if you just remove these images in Acrobat (not Reader).

You can save the document "as text".  All formatting will be removed.

How would you do it with Acrobat?

In Acrobat there is a Preflight fixup that move images, text, and graphics to separate layers. This allows you to turn the layer that contains the images off. This doesn't get rid of them, but it might be sufficient for your needs. If you want to delete the images, having them on a separate layer makes it easier to select them with the TouchUp Object tool to delete them. Unfortunately, you can't simply delete the images layer. You may also want to remove any graphics (vector) content as well.
