saving a copy of a form

hi 

i have created a form to use with work,

i need a master copy to open, fill form in and then save it as customers name, then open form and all feilds blank again ready to be used again,

i cant seem to do this in my ipad that i use all that seems to happen is it overwrittes the main copy and i have to empty it all and refill it with new data.

i hope some one can help im even looking at having an app created for this reason.

Dear Lord yes!   This is typical PC Microsoft distorted reasoning about what customers use the software for.  Maybe they could go to a seance and as Steve Jobs how to create Adobe.  Once you edit a master form, that is 'fill in the blanks', the Master Form should be saveable with the blanks empty!!
