Formular zum unterschreiben auf iPad erstellen

Ich suche nach einer Möglichkeit, dass meine Patient*innen ein von mir Erstelltes Formular auf dem Ipad ausfüllen und unterschreiben können. 

Sie sollen in den Textfeldern mit der Tastatur ihren Namen etc. eingeben. Dann sollen sie Kästchen anklicken können und zuletzt ihre Unterschrift darunter setzten in ein dasfür vorgesehenes Feld. 

Das Formular habe ich erstellt, ich finde nur keine Möglichkeit, dieses mit dem Pencil unterschreiben zu lassen. Auch kann ich es nur teilweise bearbeiten, wenn ich es am IPad Öffne. 

Gibt es dir von mir versuchten Funktionen überhaupt?

Schon eine Lösung gefunden? Ich suche auch schon lange nach genau dieser Lösung.  Aber es will nicht so recht klappen. Selbst die Online Videos sind so nicht unmsetzbar. Es kann doch nicht so schwer sein?

Acrobat Reader mobile  app provides support for the following two types of forms:

• Acroforms: These contain form fields where you can directly enter data, check boxes, and more. Users can fill the fields from any platform and can edit in Acrobat. However, you cannot edit or remove Acroform form fields on a mobile device.
  
  

• Fill & Sign forms: You can create these forms on any platform, including mobile devices. Fill & Sign form fields are similar to annotations: you can select a form element from the form tool bar and tap anywhere on the PDF to place it. Acrobat also supports adding and editing Fill & Sign form fields.
