Take picture on iphone and add to PDF form

I have built a PDF form for, very straight forward with YES/NO radio buttons, drop downs, dates, etc. The form is for supplier audits, and whilst I would normally undertake such a thing on my laptop, thought that this would also be beneficial on my iphone as I can dictate in the text fields of the document. What I am struggling with is adding an image to the desginated areas on the last page of the document where images are collected; on the laptop, this is just a browse for an image. When i tried this on the iphone there is no response, what I am trying to do is open the camera of my iphone and insert images directly; I am using Adobe reader on my iphone. I have very limted experience of javascript, but have done some in the past.

Any help would be appreciated.

How? They just built a better PDF viewer than Adobe did... It also has (much) better support for scripts.

Yeah, looked at that piece of software; I'm not ready to part with nearly £50 to have this luxury just yet. I wonder how those at PDF expert are able to do it?
