Am using acrobat Pro to create an application form for my bike club with multiple check boxes for preferred speeds, distances, etc. However I want to restrict users to selecting one of 5 check boxes.

Just give each check box the same name but different export value.

Or you can use radio buttons that are in the same group which will limit you to one select at a time.
