Extract a PDF Page and send as an Email

We have a document with the last page reserved for customer Feedback on the document. This Feedback Page has the controls and forms, such as option buttons and check box.

Process for Feedback by the Customer: The customer must give a feedback (optional) after using the document. Customer must select the options in the Feedback form and click Submit. 

Our Requirements The Feedback page must be extracted and sent to an email address specified.  Please help us to achieve this task.  Thanks in Advance!

If the file is going to be used in Adobe Reader it's not possible, only if it will used in Adobe Acrobat, exclusively.

Hi,

Does the whole page need to be extracted or just the data from the page?

    Why must the page be extracted

    1. Why is the entire file not acceptable?

    2. Why are the form field contents not acceptable?
