How to create dialog box for Dynamic Stamp

Hi, I was able to create a dynamic stamp with javascript in each field.  However, I would like to create a dialog box for multiple fields: Description, Decal#, Serial#, Bldg#.  Then when user click the apply button on this dialog box, the value of the fields will populate the stamp.  My question is how to create a dialog box with Apply button that will populate the fields in the stamp?  Thanks in advance.  I've googled and search forums with no luck.  Please point me to the right direction (what to look for) or webpage that explains this multiple fields in a dialog box, or share a sample code, please.  Thanks.
