How would I code a PDF form to merge/append a second PDF form to the end of the existing one with a button or a link that a user can click?

I have some forms developed for different types of request. There is one base form that has to be completed and about 4 or 5 others that can be added to it depending on what all needs to be requested. I would like to have a button or a link at the bottom of the base form that a user can click on to just append it.

You must add the pages from the other PDF and use this pages as templates.

You can use template pages for this.

I was doing some research on this through other forums sites and not quite sure how this works in Adobe X. I was able to find Document Processing in tools and the Page Templates option, but it doesn't appear to take the templates I make in my first form, "PDF1",  and make them available in my second form, "PDF2". Is that a possibility within the Adobe Pro X?
