Radio Button Values Change When Merging PDF files

Hello Adobe Acrobat Community,

 

I'm encountering a puzzling issue with a PDF form that contains numerous fillable fields, including radio buttons. Whenever I merge this form with another file (be it another form or a regular PDF), some of the radio buttons inexplicably change their values, while others get reset and become empty.

Has anyone experienced a similar problem? What could be causing this behavior, and how can I prevent it?

 

Each field in the form has a unique name. No identical field names.

I have identified the reason for the issue. The first and fifth columns have the same values. I resolved this by changing the value in the fifth column from 1 to 1.0001.
