Adobe Sign issues after latest Acrobat Update

Hi,

I've been experiencing several issues with Adobe Sign after updating Acrobat a few days ago. 

The most pressing issue is that the signing process is not working. 

Information is not submitted correctly, either by no info at all or by a flawed iteration of the same info and signature placement is off. (Signature blacked by me)
