Hi Everyone,
I have an issue with the adobe sign form I created. I filled out the data in the text fields, signed miself and then routed for signatures. However, the form displayes blank at the signature step, all of the entered information is gone, except my signature. I've attached a word document with screenshots to show you the visual. I'm quite new to all of this so I hope there is an easy fix. I'll appreciate it if anyone can help. Thanks.

Sorry about the delay in response.
We have checked the screenshots that you shared with us.
Would you mind sharing the step-by-step workflow you do to fill the document and send it for signature?
It seems that you are using Acrobat Pro DC to create and send the document for signature. It would be helpful if you could share the PDF form you have made. We would like to check it on our end.
Thanks,
Meenakshi

Weird, I recreated what you did in a form of my own and no information was lost when processed to request signature.

Can you share the document. Also, I wouldn't post personal information,

I replied from my email and didn't realize it would show my personal info. Is there a way for me to delete it?
The form is a regular pdf form, not a fillable form. I use it all the time. I type in info and save it, and then send it for signatures. I could share the blank form, but that probably wouldn't help, becaus usually it works, just occasionally I have an issue. I can't share the form that is filled out because that has PII.

Edit you reply above and eliminate personal information.
Since your say it happens some of the time. Do you always upload the same way when requesting signature?

I don't see an option to edit.

Clic on the 3 dots.
Then let's see what Adobe says about it, because I cannot recreate the problem you have.

When I click on the three dots, I get two options, follow or report.

I am having a similar problem. I have a form where information is typed. When I use the Require Electronic SIgnature, some of the document's text disappears from the routed for signature version. The reviewer then cancels the document because of the important missing text. Also, sometimes, light yellow boxes appear over the text and cover the text up. Deleting the light yellow box brings the text back in view. If teh light yellow box appears and the text is still visible, deleting the box deletes the text.

Any updates on this issue and how we can solve it? As soon as I download the newest version of adobe acrobat I can no longer send my prefilled fillable PDF forms for signatures. I filled everything out save it then send it for signatures and all of the Fields that I filled out disappear, that did not happen before the update and now I have no idea what to do.

Hi sarahjbirdwell,
Thank you for reaching out.
This has already been reported to the team, and they are working on the fix. Could you please share the Acrobat and OS version on the machine?
Thanks,
Meenakshi

Hi, I, likewise, would like a resolution update regarding this problem.
I've now encountered it when receiving a filled pdf form whichI then route for signature and when filling the same form from scratch and routing.

Has this been fixed yet? I have some important docs to send out.

I am running into the same issue. Half of my form boxes come over, half disappear. When i exit e-signature the forms are also removed like I never made them. Assuming this has not been fixed yet?

I am having a similar experience where I cannot see the tags on the places that are tagged for signature, date, initials, etc. after creating and sending the document out for signature.

I'm having a similar issue where the button that I've created during Prepare Form is no longer displaying when I request an e-signature. Have you encountered this issue and, if so, did you find a resolution?

Thank you for reaching out.
Please ensure that you select the "Prepare form for e-signing" and then add the radio button option. When you prepare a form in Acrobat, all the fields are not supported on Acrobat Sign. Hence, there are separate form fields for Acrobat Sign documents. You may use the following steps when adding form fields in Acrobat to send for signature:
- Open the PDF and click on the Prepare form.
- Then check the box for "This document requires signature". Check the screenshot for reference:
- Then click on the Create form button.
In case it opens directly on the next page, click on the three dots in the top right corner of the left-hand tools pane. Then select "Prepare form for e-signing". Check the screenshot for reference.
For more information on the Preparing the form, refer to the following help document: https://adobe.ly/4dQYXfw.
Let us know how it goes.
Thanks,
Meenakshi
