How do I setup a widget that I don't sign but two other parties do?

I would like to create a widget that goes first to a generic person (different person every time) who clicks on the widget link then completes the form and then have it go to another person (who is the same signer every time) for the final signature and they both automatically get copies of the completed agreement along with my company.
