Signature fields missing when document gets sent

Hi - I'm working on a document that has signature fields missing when they get to the "signor."  My template shows the fields correctly, and the fields preview correctly before sending the document.  When the document is retrieved for signature, it's missing almost half of the normal fields.  Does anyone know how I can fix this problem?  Many thanks!

Can you inbox me the document details such as Name, date, sender's email address so that I can further check? please don't share it on public forum.

Rijul,

The document name is Personal Concierge Agreement. I have put the

signature/date/etc. boxes on the lines where my clients need to sign

without a problem. However, when I send it, they don't all show up as

needing to be signed. I have re-uploaded the document a few times to start

fresh, but the same thing is happening. Sarah

I'm getting the same problem. This looks very unprofessional. It would be nice if Adobe could create a simple, easy to use product that just worked. I'm getting very tired of dealing with these stupid bugs, lack of obvious features, poor customer support, etc. I see that they never replied back to you. They don't reply to my inquiries either and there is no phone support for non sales issues. Grrrr.
