Unable to digitally sign PDF

I have created a sign-in sheet [Monthly Safety Meeting.pdf] for office personnel. Most people are able to digitally sign these PDFs. One person is unable to sign any of MY PDFs. Yet she is able to sign other PDFs (that I have not created).  Also, she recieved report codes 1002 and 1008. Other personnel have not reported such  problems or report codes.
