In a web page project when you add an image, you should see a gear icon in the bottom right of the image. Click it to get a window where you enter alt text:

From Adobe Express, you could download the image as a PDF that you could then make accessible with alt text:

 

Generally, though, alt text is added to an image where it is being placed (web page, email, PDF). Otherwise, data that would be a part of the image itself would be metadata. You can add metadata via Adobe Bridge or Photoshop:
