All 4 outcomes of a prompt are identical

The outcomes of my prompts are all the same exact thing, there are no variations. Is it something I do? What can be done about that?

Adding to what @droopydog500 has said, a screenshot would help explain what has happened. If you have been given 4 options that are 100% identical, that is something that should not happen,
Also share what browser you are using, including version.

    Is this happening with every prompt or did it just happen on one specific one?  Can you please show us a screen shot?
