When I download, it downloads the original image and not the adjusted image

I presume you are using generative fill. Before downloading, did you press the "Keep" button:

If not, you need to save the change with the keep button before saving the image. If you did press keep, please let us know what kind of device you are using along with the operating system version and browser.
