Unable to initalize a flash form. The mxml.cfswf file is returing a 404.

I am running ColdFusion 11 on IIS 8.5 and attempting to display a simple flex form.  It generates the infrastruction for the form in the browser.  However, when it attempts to access the <unique number>.mxml.cfswf file I see a 404 in the IIS log.

Any help in troubleshooting this issue would be appreciated.

I was able to resolve my issue.  The servlet was not enabled in the web.xml file.  Once I enabled the servlet and restarted the ColdFusion Applicaiton Server service the page initalized.
