I am having this same problem. I have checked to make sure that I am sending ACSM files to ADE (not overdrive), and the problem continues. I work at a public library, and one of our customers came in with this problem and I couldn't help her. I spent an hour an a half trying ALL of the above solutions, with no luck. I imagine that other users are having the same problem. What do I tell my customers who are having this issue?

I have found that if I authorize a book that was already working, with the same user ID that it now no longer works either
