CS6 uninstall and use Cleaner Tool, and re-install CS6.

Detailed symptoms please? Messages etc. No personal info. Do not post serial numbers. Also your system version.

We have box version and others licences by subscription. And we don't know what date we need to fill in this form, we can't understand where we can specify our license key.

You cannot mix CS6 box version with CC subscription on the same computer.
