How Fix "Plug in Failures"? | Flash

I have tired un-installing and re-installing flash and it has not help. Keep getting "plug in failures" , large exclamation marks with loud buzzing sound,  also

caused my computer to freeze and can not click on anything.This has been happening for over a month. I have no Extensions on Safari.It seems to happen randomly on any website that uses flash player.

 

I have a Mac OS X (MacBook Pro)

El Capitan Verison 10.11.4  but had Yosemite in the past with same problem

Safari 9.1

NPAPI Plug in Version 21.0.0.226

PPAPI Plug in Version 21.0.0.213 installed

    Flash Player 21.0.0.226 is not the latest version.  The latest version is 21.0.0.242.  Please download and install the latest version and see if the issue persists.

    The PPAPI version is for Chromium-based browsers.  If you only use Safari there's no need to have it installed.  It doesn't hurt, but not necessary.  If you do use a Chromium-based browser that supports Flash Player (e.g. Opera),  you'll also want to install the latest version, 21.0.0.242.
