New file window looks weird

When I click on new file, instead of showing something like this:

It looks like this:

I can't figure out how to fix this, and it's also the most recent version of illustrator.

That is in Preferences > General: Legacy New File Dialog (you need to turn it off to get what you want)

Looks like you have checked: Preferences > General > Use legacy "File New" interface.
