Check validity ePub TOC

Hi,
I received a message from Apple that one of the books we have in the iTunes store:
"Incomplete/Inaccurate app generated Table of Contents".

I can't figure out:
1. what the problem exactly is, and
2. how to set the export settings of my InDesign book to a FLO epub in a way that it the TOC passes the iTunes check.

All help is very much appreciated!

You might have the problem here, but I don't know. The truth is, I didn't use the InDesign TOC function. The document was created first as a PDF publication. I the TOC in the beginning the page numbers are linked to text anchors on the respective chapter pages. This works totally fine in a PDF, but apperently the ePub had a problem with this.
I have never uses the InDesign TOC function and I'm ok with trying this out if you think this indeed does the trick for the iTunes validation...

I guess you nailed it here, Jonathan 😞  I remember I was struggling with getting the TOC right when I started on the book. I skipped an issue I had by creating it manually, which now probably is causing this epub validity problem. With the help of the instruction video I dived into it again and I've almost created a copy of my manual version. There's one odd thing that I can't solve. I don;t know if you're a TOC expeert yourself, but in case you still have a tip that would be great. 
After the (correct) page numbers a fixed extra nr. (15) is inserted everywhere and I can't find out what causes this....


alright!! now we cooking with gas, sounds like the issue you posted about what solved and now we need to look at the double entry you getting? seems like there is somethign in page 15 for sure.

lets take a look at how you set up the TOC. screenshots?
