I have a csv file that has cantact data and images.  I am trying to get the data to populate four columns with no luck.  I lucked up several days ago, but I can not repeat four columns of data

You have to create the columns first, because that is what InDeisgn uses to populate the page with if you use Data Merge with Multiple Records. You only have to place one text frame though. See the screen shots.
