Endnotes not starting anew

I am having a lot of trouble with endnotes. First they didn't show up in the flowed text at all (I had to create a new document, where they mysteriously DID appear, then copy and paste them into the original). At the end of the document they appear as <?> instead of numbers or the number 10 is repeated a few times. I went through and fixed the endnotes by hand. Now however I realize the in text references are all wrong. It looks as if they are in sequence according to endnotes from previous stories in the document, instead of beginning over. They correctly began with "1" in the original Pages document (and correctly showed up there, too). Why would all this happen? It never did before.

How did you place the Word doc originally?

I believe I created a text box, clicked Command + D, and selected the document to be placed.

Did you turn on "Show Import Options" and have a look at what is on and off in that dialog box?
