Feedback form in an epub

Hi,

Is it possible to add in a feedback form in an ePub?  I have created my document and with videos and added buttons with checkboxes and submit but struggling. Can get form to work as interactive pdf but need this to be in an epub. Any help will be appreciated.

You can't reliably have videos playing in an Interactive PDF, you can include a hyperlink in a PDF to a YouTube, or similar, external video.  Use simple hyperlinks, no multi-state buttons.

You can't have an interactive form in a FXL ePub or a Reflowable ePub. 

You can include a Google Form with the InDesign plug-in in5 (at extra cost) this video shows you how: https://www.youtube.com/watch?v=UgDAg5mTW9Q
