Formatting Endnotes in InDesign

Question

In my InDesign document, the end notes are all hyperlinks but only some of the end notes are showing as hyperlinks (See screenshot).  Also - I want to get the URLs to wrap flush with the first line rather than flush left.  I've searched everywhere and can't seem to find a way to make this happen.  Any help would be appreciated.
