Hi.
I've created an interactive form, where the "check boxes" and "text fields" seem to be engaging / working ok.
However I'm trying to assign a "Submit" box, where users will submit the form to my email address.
Firstly, is this possible?
Secondly, please see my screens attached and advise if you can see something I've done incorrectly?

My standard advice: Abondon this project right now and create a proper HTML based form. This can be done using any number of tools or even a service such as Google Forms depending on your website.

Did you add 'mailto:' before your e-mail address in the field in InDesign? Don't use buttons, but create a hyperlink and then use the e-mail with 'mailto:' (Buttons do not work in PDF)

But that won't attach the PDF, it will just try to open the default email application.

True. I misread. Still, I suspect the OP forgot the 'mailto:' part?

I haven't used that in ages and when I did, it was back when you had to do so in Acrobat so I don't even remember how it worked.
