Table of Contents entry missing leader dots

Only one instance of a TOC entry fails to produce the leader dots. It shouldn't be a paragraph style issue, since the "Notes" section is the same style as all the other entries that work correctly. For example, the "Appendix" chapter is the same paragraph style, which generates the leader dots correctly.

Has anyone seen this before?

Is that paragraph style overridden in any way?

When you make a ToC, do you also save and name a style preset?

The style doesn't appear to be overridden. The "clear overrides" icon is greyed out:
