In your current case - you don't need more than 3 levels - unless you have some other lists.
You've also mixed Graphic with Title - they need to be in the same "scope":
You also NEED to Anchor your images - or rather group of Image and Caption - otherwise, InDesign won't be able to index them properly.

Unfortunately, you are limited to 9 levels. If that's not enough - then you need to split your document into smaller portions and use Book feature.

You can have only ONE Chapter per INDD Document - that's why you need to use Book option:

No--you can't do it without using a book. A single file can only have one chapter number. If you try to make a complicated number sequence, the number style (00) would apply to all auto numbers. You should be able to do this if you don't mind your pseudo-chapter numbers showing up as 01, 02, 03.
I would use a book file--it's not hard to learn if you don't already know it and it makes your project much easier to work with.

You have to use a book file to do what you want. It's not that hard to learn. You would export a complete PDF from the book that would include all chapters.
