Hi guys,
in the past I used to have a very simple action in PS that I made myself: resize the image and 'save' on desktop and close the file.
yesterday I had to reinstall PS that caused my action not to work anymore, and since it's quite a simple one I made it again (I do tend to use it a lot).
now... what happens is that every single picture that I run with this action is saved on the desktop as the file that I used to make the action... the picture is different, but the name is the same....
in the past I had every file that I ran with the action on my desktop, but now it's just one file, showing the same image name over and over again... very annoying and I just don't remember what I have to do...
in the text of the action it shows the name of the file, so I know that it goes wrong at that point, but... what do I do to avoid that?

Actions can either record the filename + path, or just the path. Don't change, touch or even look sideways at the name field when recording the save step, just navigate to the required save path.
Your issue lies in the circled section. It will consistently save files with that specific name, which may lead to overwriting files that share the same name. Record the action as demonstrated and clarified by @Stephen Marsh.

I tried making the action a few times over and now it works just fine, without the filename in it..

I already figured out that this was the problem, but I could not change it in the action itself...
I tried making the action again and again a few times and finally it did what I needed, so all is good now! don't know if or what I did differently though... but it works and that's just great!
