Reproducing this: Save a color-managed PSD and uncheck the ICC Profile (see screenshot). Go to save as to save the file again, or go to save a copy and save a .jpg, and the ICC is rechecked. If I change the file settings to do not manage, the box is unchecked every time instead of checked every time, regardless of the last. This makes it much more confusing and harder to keep track of to make sure I'm saving things correctly. Previously it would maintain the color settings from the last file you saved from the document.

Hmm. That check box should be sticky based on it's last state.
Let's make sure we're in a default state and there are no stale setting somewhere:

Does it work correctly?
If that doesn't solve it, you can quit Photoshop and put the Settings folder back.
If it does solve the problem, I would like to get the folder of settings to try and figure out what is bad with them.
