Measurement tool confusion

I have units and rulers set to inches. When I measure one inch with the ruler, the counter says "240." I'm confused.

You have "Use measurement scale" checked.

Go to Image >Analysis >Set measurement scale and check what scale is being used, by default it will use 1 unit per pixel i.e. will measure in pixels,  or just uncheck it and it should show in inches as set by your ruler
