Photoshop brush preview problem

The brush option through F5 works very well however, the problem is that the brush preview brush stroke in the top toolbar has not been applied.

Seems to work here, what have you done for trouble-shooting so far? 

Restoring Preferences after making sure all customized presets like Actions, Patterns, Brushes etc. have been saved might be a good starting point:
