NeatVideo De-Noise error "Premiere has not provided input frames from the video"

Using NeatVideo's Reduce Noise v5 Video Effects plugin. I'm getting an error after executing Prepare and Build the profile -- and the build fails.  The error message has several possible causes (shown in the error message), but I cannot correct the problem.  Neat tech support suggests it's a Premiere problem.
