are you sure that the metadata is compatible with the mp4 format? I'm only seeing these options in the export dialog: metadata. You may need to use a different export format to include other Metadata at least in Premiere...
You can add some metadata information as a burn in window using the video effect: Metadata and Timecode Burn-in
It's possible there are other options that I'm not aware of...

We are adding the metadata in certain fields while exporting. I am giving those fields given below :-
In Dublin Core Metadata section, we are filling the fields :-
Contributor : Name of the people involved in the project.
Description :- A short description about the video.
Identifier :- Serial Number of project.
Copyright Notice :- Copyright details
Title : Title of the project
and in the Rights Management Metadata Section :
Owner : Owner of the Video/Project
Web Statement : Website URL of company
and, we are exporting it in .H264 Format and the output comes as .MP4 File why is this comes like this in .MP4 format is my first question. The metadata is comes in an embedded format within the video. After exporting the video, I opened the file in MediaInfo program to view the metadata. But I can't view all the fields of Metadata in the MediaInfo program. I tried different views but I failed. But, when I export the file in a Quicktime format the metadata comes in a seperate notepad file, where I am able to read all the metadata fields which I filled before exporting the video. Why there is a difference between metadata reading between .H264 Format and Quicktime Format ? Please tell me how to solve the metadata exporting issue related to .H264 Format . I am giving a screenshot of my file export details
