Video color is offset rgb

Hi, I'm working with premiere cc and this just started happening to any video I import to edit (see link). Looks like the colors are being separated and offset along with those bands running across the screen. The raw video is fine when I play it and sometimes it's nornmal when I try exporting clips from it. I don't remember changing any setting or adding any filters/effects.
