The effect is (or rather was) designed to reduce defects in the image itself, not the content.

I often film products made of glass or metal.
There are minor scratches. Dust & scratches has removed those scratches and not blurred them, even without selecting every single item.

Or even dust was removed without blurring and you didn't have to select every dust particle.

I agree with you. I've even used that effect maybe five times in the last decade. But the reality is, like a lot of my color control/mod requests, the user subset that would actually use this is ... relatively tiny.

At which point the devs are going to note that there are excellent plugins for this rather niche workflow. Which is understandable, if ... well ... yea, irritating.

I wish it were otherwise. But as a practical person, there are some excellent plugins for helping with this. Check out aescripts.com first, then there's a whole list of companies doing Premiere effects plugins.

Yes, but this is also an another effect.

With the dust and Scratches effect you can remove small Trash and debris without blurring the frame so much.

Denoise is good for grit

Tried VR:de-noise?

Since the new version 24.6, the “dust & scratches” effect no longer exists. Adobe says it has been replaced with gaussian blur. But they are two different effects! gaussian blur simply makes things blurry. Who has a solution? How do you replace the effect?
