Video plays back as colored static

I've had a problem with this since CC 2015.4 (I've been using 2015.3 instead) where sequences with h264 clips play back as green static or static of other assorted colors. Prores clips have fewer problems, but if there are effects or motion effects added it'll also appear as colored static.

I captured playback of a few sequences as an example

1. All h264 clips

2. Prores 422 clips, some with effects applied

3. Prores 4444 clips, then switching to original h264 clips

Has anyone seen this or know what the problem might be?

I'm on a 2010 Mac Pro with a Radeon 5770 card for the main display, and an added Quadro 4000

    As a test, go to File > Project Settings > General and set the Renderer to Software Only.

    That did it! For some reason in the newer versions of Premiere the preference changed to OpenCL. On CUDA or Software Only it works fine. I rarely deal with that preference, so it hadn't occurred to me at all.
