I'm having the same problem with three links in two topics in my integrated files (TCS2/FM9/RH8). I've tried deleting and re-adding the cross-refs in the FM, putting them in a different location, using a different cross-ref format, forcing updates, etc. and nothing seems to work.
I've attached a Word doc with some screen captures of one of the problem topics. The first one shows the text in FM. The second shows the RH Design view of the imported topic with the problem link in the first bullet. The third shows the HTML view of the topic content and the absence of the href info.
All links in the topic use the same cross-ref format. All cross-refs are mapped the same way for conversion. I tried redoing the links with a different cross-ref format from the rest of the links in the topic, and they still don't convert properly.
As for the comment that this situation is only possible if the topic was open in RH when I did the update, I made sure the topic was not open (no topics were open in RH). And the topic isn't a large one. I waited 5 min after the update completed before looking to see if the update had worked (it didn't).
So now I have these three links that are broken in RH. Is there a way to fix these in FM? If not, can I fix them manually in RH before generating the Help? I realize that I'd have to do this each time I regenerate the Help, but it's better than having broken links in the output.
Thanks for any and all help with this issue! It's been driving me nuts for a week now.

I don't have a fix yet, but I do know why my links are broken. During the linking/update process, Robo is not creating two topics. In both cases, the missing info is what should be the first topic created from a chapter file. For all other chapters, all text is being turned into topics appropriately, according to my pagination settings. I can't figure out why this text is being ignored in two cases only, since it is formatted the same way in all chapters. I can see no difference in formatting between the problem chapters and the working ones. I've opened a case with Adobe and will update when/if I learn more.
