Missing Books

I have compiled a help document that, for the most part, functions as expected. The problem, however, is that there are several books listed in the TOC when I am editing the help that simply do not appear after I have compiled the help. Generally, I compile as HTML help, and I essentially have to follow this convention because my output must be Vista compatible. Is there something that could potentially be removing these books from the TOC? As I search through the output, I also find that the topics references by the TOC do not exist. Any thoughts/suggestions?

Update on this problem: it seems to exist only when compiling to a chm. I tried recompiling to winhelp, and the books/topics compiled directly. Anyone else have thoughts or ideas as to why this is happening? I would really appreciate some feedback.
