Hello,
I need your help with the image rights authorization form for adults or children. I have uploaded several completed and signed JPG forms but they have all been refused.
Can you help me understand the reason for the refusal?

You really have to fill in all the required fields and have them signed by everyone: the model, a parent, in the case of a child, and you and a witness.

Besides the good advices here: pay really attention that all the parties sign the same date.

If you really dont get the error, post a dummy model release or a model release with private data blacked out. But generally, all what can go wrong has been told here.

I agree that you should read and reread the requirements, you must follow them exactly. I can say that a common fault with the forms is not following the rules for dates.
