Property release asked for; first one I sent has been rejected

I took the picture of these dolls. The dolls were made by my sister-in-law. Adobe asked for a Property Release. I filled it out in the following way, which was rejected (they asked me to resubmit the Property Release):

Property Owner: myself with my personal information (individual owner)

Property Information: a description of the dolls and the address of the artist.

Artist Information: dolls makers signature, name and date signed

Witness: signature, name and date signed

Recording session: Description & Location: "Picture taken at home

Date of Recording Session(s): 7/19/2019

 

Can someone point out what I am doing wrong? Thanks so much.

So far as I understand it:

The owner is the IP owner, that is your sister-in-law. You own the object, not the rights, and your ownership is irrelevant from a rights point of view. [Unless you have paperwork transferring the ownership of all rights to you].

The artist is the photographer: you.

Yes. I used the address of the artist.
